Models, with access and effort by default

Organization → Models brings Model access, one row per model with Enabled, Maximum effort (No limit, a level or Not available) and Available to, and below it the default model, with Use Nouter’s recommended defaults or Choose a specific model and effort level, plus the switch to always start with that model and effort.

The chat picker shows the model and, in a submenu, each model’s effort level, capped at that limit.
